People recover better and faster when they’re in their own surroundings, so we’ll discharge you from the hospital as soon as possible. But if you still need help with the daily tasks like fixing food or taking a shower after a hospital stay, we can help. Here’s how it works:

• Before you’re discharged, our Social Services Department will consult with your caregivers.
• They’ll discuss how much help you’ll need to live safely at home, while regaining your strength.
• Our home health care partner, ConnectCare, will assign a registered nurse, licensed practical nurse, aide or therapist to visit you at home, depending on your needs.
• When they visit you at home, ConnectCare staff will help you get stronger, learn new ways to care for yourself and work with you to regain your best quality of life.

ConnectCare also provides services to the homebound, and to those who just need a little help to stay in their own homes.
