Your cardiac rehab journey at GRHS begins with a referral by your health care provider. Then, a cardiac rehab and heart failure nurse will review your history and conduct a medical evaluation. She will develop a program specific to your needs that includes an exercise plan, and will introduce you to valuable resources to help you in your recovery, such as weight loss and smoking cessation support. You will also have the opportunity to meet one-on-one with our registered dietitian to discuss your eating habits and changes that can positively affect your health.
